With 35 in-network skilled nursing facilities (SNFs), approximately 76,000 patients attributed to Medicare Shared Savings Programs, and nearly 520,000 patients attributed to Atlantic Health’s three accountable care organizations, ensuring positive patient outcomes post-discharge is critical to Atlantic Health’s success as it moves into value-based risk arrangements.

Now, CarePort Connect empowers Atlantic Health providers with real-time data and visibility as patients journey across the care continuum and facilitates electronic sharing of documents and communication between acute and post-acute settings.
